Collaboration is essential to tackling today’s public health challenges — because no single organization or sector can do it alone. This is why relationship building and partnerships are at the heart of our work to support healthier communities. In this ongoing series, we spotlight the individuals, organizations, and collaboratives that make up Trailhead’s network of partners, sharing their impact in their own words.

Who do you or your organization serve?

We are a training program for health/patient navigators, Community Health Workers (CHWs), promotores de salud, care coordinators and their supervisors.

Which communities or regions do you support?

Throughout Colorado and nationally. Our courses are offered both in-person and online, which increases our geographic reach.

What’s important about this work for your community?

We provide professional development for the CHW/navigator workforce so they can better serve their communities and reduce barriers to care. Studies have shown that CHWs help reduce health disparities by providing culturally relevant services and linking clients to resources.

PNCT is a state-recognized training program, which means our curriculum is approved by the state health department. Individuals who receive training and pass an assessment receive a CHW credential and will be eligible for Medicaid reimbursement in 2025.

How can people support your work or get involved?

  • Visit our website and sign up for our monthly newsletter! We also have opportunities for organizations to host a CHW intern or apprentice. 

Through fiscal sponsorship, how have you been able to increase your impact? Or how has your work changed or evolved without having to deal with administrative burdens? 

We contract with a network of subject matter experts and trainers with experience in the field to deliver our curriculum. Trailhead is essential in allowing us to pay contractors on a timely basis and the ability to set up fee-for-service contracts swiftly. Our program’s core management team is small, so we appreciate the administrative support that Trailhead provides. They are a key partner in a CHW training and development grant from the Health Resources and Services Administration (HRSA).
