Regional Health Connectors work across every region of Colorado, fostering connections between clinical and community resources to address the unique health needs of their communities. They also support practice transformation efforts that support practices in improving quality of care while enhancing the opportunity to succeed in new payment models. 

Where Regional Health Connectors Work

The 21 Colorado Health Statistics Regions are aggregations of counties developed by the Colorado Department of Public Health and Environment’s Health Statistics Program in partnership with state and local public health professionals. RHCs are locally-based within each region and serve the communities where they live, work, learn, and play.
